Please complete our adoption application below as thoroughly as possible for consideration. Upon submission you receive a confirmation email that you MUST RESPOND TO for your submission to be complete.
Please read the following statements below, your submission of the application acknowledges your agreement to the following:
- I am certifying that all of the answers below are true and accurate to the best of my knowledge.
- I intend to be the legal owner and person responsible for the care and well being of any pet I adopt from Better Days Animal League.
- I give any representative of Better Days Animal League permission to contact any entity or person listed on this application to assist in making a determination to place a pet with me.
- I authorize any entity or person listed on this application to release information about me by phone, print or electronic medium to any member of Better Days Animal League.
- I agree to allow, if necessary, a home visit by appointment before or after placement of a animal by a member of Better Days Animal League.
- I am prepared to meet the expense of caring for this animal.
- I am familiar with the laws concerning rabies vaccinations for dogs and cats.
- I am familiar with dog leash and licensing laws.
- I agree that communications will be via email.
